Repeat inappropriate defibrillator discharges following defibrillator implantation. What is the mechanism and treatment?

چکیده
Introduction Inappropriate shocks from implantable cardioverterdefibrillator (ICD) devices are a well-described clinical problem that contributes to patient morbidity and mortality. There are a number of causes for inappropriate ICD discharge, and T-wave oversensing (TWOS) is one such example. Improvements in ICD sensing algorithms have reduced the incidence of inappropriate therapies but have not eliminated this clinical problem. In this article we present an unusual cause of TWOS leading to multiple inappropriate ICD discharges. Furthermore, we review the algorithms in use by the major device manufacturers to help reduce the incidence of TWOS.

Implantable cardioverter-defibrillator in a patient with dextrocardia situs inversus
Background: Dextrocardia is a congenital anomaly, which may have coexistent coronary artery disease (CAD), arrhythmias and conventional indications for device therapy. However, the implantation of transvenous leads can be technically challenging and the approach needs to be tailored to the patient's individual anatomy. Case presentation: A 54-year-old male with dextrocardia situs inversus and i...

Implantable Cardioverter-defibrillator Therapy for Hypertrophic Cardiomyopathy: Usefulness in Primary and Secondary Prevention.
INTRODUCTION AND OBJECTIVES Hypertrophic cardiomyopathy is a frequent cause of sudden death. Clinical practice guidelines indicate defibrillator implantation for primary prevention in patients with 1 or more risk factors and for secondary prevention in patients with a history of aborted sudden death or sustained ventricular arrhythmias. The aim of the present study was to analyze the follow-up ...
متن کاملQuality of life in patients with an implantable cardioverter- defibrillator – the significance of clinical factors
Introduction: Implantable cardioverter-defibrillators (ICD) in patients exposed to the risk of lethal malignant ventricular arrhythmia proved to be the most effective method for preventing sudden cardiac death. The remaining problem is the tolerance of painful ICD discharges and patients’ quality of life (QOL). The results of the study on quality of life after implantation of an ICD were unequi...
متن کاملDevelopment of ventricular fibrillation after implantation of a biventricular implantable cardioverter defibrillator: what is the mechanism?
Syncopal spells in heart failure patient with cardiovascular implantable electronic devices (CIED) require multiple assessments. T-wave oversensing is a well-described phenomenon that remains significant in modern implantable cardioverter defibrillators (ICD) systems. It can lead to inappropriate therapies and loss of biventricular pacing in those with cardiac resynchronization devices. Strateg...
